T-wave Oversensing and Symptomatic Bradycardia in a Pacemaker-Dependent Heart Failure Patient due to Drug-induced Hyperkalemia

چکیده
Hyperkalemia is a common clinical condition among heart failure patients especially if there coexisting older age, potassium-sparing medication, and renal insufficiency. Close follow-up titration of renin-angiotensin-aldosterone system inhibitors are essential to prevent the development hyperkalemia. It can also cause significant abnormalities in with cardiac implantable electronic devices (CIED) including pacemakers cardioverter defibrillators (ICD) due its impact on electrophysiology. Herein, we presented pacemaker-dependent patient single-chamber ICD whom bradyarrhythmia signs symptoms were observed T-wave oversensing managed appropriately.

A case of inappropriate implantable cardioverter defibrillator therapy induced by T-wave oversensing due to hyperkalemia.
There have been reports of hyperkalemia-induced T-wave oversensing in patients with implantable cardioverter defibrillators (ICDs). However, a comparison of T-wave amplitudes and morphologies between the surface 12-lead electrocardiogram (ECG) and ICD electrogram has not been reported. We present the case of a 70-year-old man who received inappropriate ICD shocks due to hyperkalemia-induced T-w...
متن کاملInappropriate shocks by subcutaneous implantable cardioverter-defibrillator due to T-wave oversensing in hyperkalemia leading to ventricular fibrillation
Introduction The implantable cardioverter-defibrillator (ICD) has reduced mortality in survivors of sudden cardiac arrest and patients at high risk of sudden cardiac death. This benefit comes at the cost of device-related complications, including those related to transvenous leads. The subcutaneous ICD (S-ICD) is an alternative to the conventional transvenous ICD (TV-ICD) system and has no tran...
متن کاملhyperglycemia-induced t-wave oversensing as a cause of cardiac resynchronization therapy (crt) failure
t-wave oversensing occurs when the counter starts giving dual beeps for every cardiac cycle instead of one. this usually happens when the monitoring lead displays a tall t wave, which is also sharp. r wave sensing algorithms of the devices do not sense t wave because the slow rate of the t wave is much less than that of the r wave. but the slow rate of t waves may change with time and also beca...
متن کاملTemporary severe bradycardia due to pacemaker programming
A 67-year-old male patient received a pacemaker for syncope. His parameters at check-up were: P/R amplitude 2.8/11 mV and atrial/ventricular threshold 0.875/0.625. The pacemaker was programmed as follows: lower rate 60 bpm; upper tracking rate 130 bpm; sensed AV interval 120 ms; paced AV interval 50 ms; atrial/ventricular sensitivity 0.5/5.6 mV, and atrial/ventricular output 2/2 V. Due to synco...
متن کاملT Wave Oversensing
T wave oversensing is quite a familiar phenomenon to anyone who has watched cardiac monitors for sufficiently long periods in the coronary care unit. The counter starts giving dual beeps for every cardiac cycle instead of one. If it persists during several cycles, the displayed rate goes up and the monitor sounds an alarm indicating 'tachycardia'. This occurs usually when the monitoring lead di...
